Using Google Docs for Formative Assessment

Using Google Docs., this form allows me to circulate the room and create a snapshot in time based upon my observation of knowledge, skills, and habits of mind students are demonstrating. The data I record can be used to reflect upon my teaching as well as have one on one meetings with students about their progress. The spreadsheet side of this form allows me to manipulate data to meet my goals! Quick, Easy, and Effective!  Developed this idea with my teaching partner Ted Malefyt and input from my dad who was doing this before they had a name for it!
